We understand how important it is to find the right home for your or your loved one. To help you navigate through the process of accessing residential aged care, we’ve created the following five-step guide. If you’d like us to walk through this process with you, we’re here to help, every step of the way.

Step 1

Obtain an assessment by the Aged Care Assessment Team (ACAT) to determine if you are eligible to enter into any residential aged care centre.

Referrals for an ACAT assessment can be made by anyone. Information about the ACAT closest to you is available by calling the My Aged Care phone line on 1800 200 422.

Step 2

Contact us and take a tour of your preferred Aged Care Plus Centre

Deciding which aged care centre is best for you is an important and personal decision. We recommend that you visit your preferred Aged Care Plus Centre so that you can experience the loving care we provide and meet the dedicated team of carers who will work with you to ensure your care needs are met.  Please bring your Aged Care Client Record (ACCR) with you if you have been assessed by ACAT.

At the conclusion of the tour we will go through the information pack and explain the fees and charges.

You can call your preferred Centre direct, or if you are unsure of which Aged Care Plus Centre will be best for you, call our enquiry line on 1300 111 227.

Step 3

Complete and return your application form to Aged Care Plus with all of the required documentation

Once you have experienced the care we provide, met the care team and are ready to move into our Centre, there are four (4) pieces of information that must be completed and returned:

  1. Application Form – completed by the applicant or representative
  2. Aged Care Client Record (ACCR) provided by the ACAT
  3. Assets Assessment – A full copy of the Assets Assessment notification from Centrelink/DVA is required if you are wanting to enter one of our Centres as a fully or partly supported resident.
  4. Certified Copy of Power of Attorney, Enduring Power of Attorney or Guardianship

If you are not ready for placement, you are welcome to return your completed Application Form to join our waitlist.

Step 4

Complete and return your application form to Aged Care Plus with all of the required documentation

Step 5

Move into your preferred Aged Care Plus Centre.

Once we have determined that we can meet your care needs and have a suitable vacancy, we will make a formal offer to you for the room and confirm with you a move in date.
